BGSI (Biomedical and Genome Science Initiative) is Indonesia’s national effort to develop a secure, scalable, and equitable genomic infrastructure managed by Balai Besar Biomedis dan Genomika Kesehatan (BB Binomika) under the Ministry of Health. BGSI advances precision medicine by building end-to-end systems for genomic data collection, analysis, and responsible data use, designed to be expandable, inclusive, interoperable, and aligned with the national digital health platform SATUSEHAT. BGSI recruits participants through a nationwide hub and spoke network connecting hospitals, spokes, and satellite centers coordinated by MoH vertical hospitals including RSCM, RS PON, RS Kanker Dharmais, RSUP Persahabatan, RSPI Prof. Sulianti Saroso, RS Sardjito, RS Ngoerah, RSAB Harapan Kita, RSJPD Harapan Kita, and RS Marzoeki Mahdi. Participants include hospital-based patients with specific conditions and general population participants enrolled in GENERASI SEHAT Program. Samples and data are transported to the national biobank and sequencing facility at BB Binomika for standardized processing and analysis, while hub sites focus on recruitment, sample collection, and clinical workflows. This model ensures quality and consistency while enabling broad national participation. Over the past two years, BGSI has completed more than 10,000 human whole genome sequences, built genomics capacity across major hospitals, and conducted population-level pharmacogenomics analyses to explore genetic factors influencing medication response. BGSI continues to expand partnerships, infrastructure, and capabilities to support research, clinical implementation, and the transformation toward a more resilient, data-driven, and precision-ready health system for Indonesia.
