Provide education related to diabetes management; pathology, medication, prevention, nutrition, lifestyle behaviours, monitoring and self-management support. Ongoing independent assessment of large caseload, preparing, planning, delivering and documenting sessions. Review blood glucose data for the purposes of evidence-based behaviour and medication adjustment according to the Clinical Practice Guidelines. Participate in the development and implementation of community health programs.
• Provide in-depth insulin pump and ancillaries training to relevant HCPs and patients in support of Territory Manager’s training needs • Educate patients and caregivers on essential topics of diabetes management including basal/bolus therapy, carb counting, ketone testing, sick day management, and troubleshooting hyperglycemia and hypoglycemia in self-management of blood glucose • Collaborate with patients' primary diabetes care providers to supply accessible and digestible education adjusted to patients' health literacy building therapeutic rapport with patients and HCPs to ensure positive patient outcomes on Tandem products • Enhance delivery of patient-centred, holistic diabetes care
• Educate & advise patients on diabetes self management in individual and group settings • Collaborate with multidisciplinary team to develop care plans for patients with diabetes • Teach self-monitoring of blood glucose (SMBG) • Teach administration, titration and management of insulin • Provide consultation as requested to community groups, partners and team members • Effectively communicate to patients, families, caregivers and healthcare professionals • Participate in disseminating the DEP vision – act as a Diabetes Ambassador • Deliver practical diabetes education in partnership with Diabetes Depot • Training of additional staff and any new educators, as required
