Improving access to mental health services in Family shelters and assist families with children in shelters as they navigate multiple systems and cope with the stress anxiety induced by homelessness. Client Care Coordinator Responsibilities: • Overall delivery and coordination of mental health and related services for homeless families residing in contracted families with children shelters. • Completion of biopsychosocial assessments. • Multi-disciplinary team approach to service provision • Providing information and training to other members of the team around client’s psychosocial stressors and needs to ensure positive outcomes for families. • Familiarity with substance use disorders and addiction treatment services, domestic violence, child abuse and maltreatment, trauma and mental health conditions. • Work from a strengths-based, family-focused, client-centered perspective and identify and fortify the strengths that each family has. • Assist families with specific stressors associated with living in shelter • Ensure that families are receiving full support in shelter and, when necessary, refer families for counseling or other services • Confer and consult with professional and technical personnel in implementing a multidisciplinary approach to client care and well-being • Serve as agency/program liaison to community agencies and/or groups • Develop linkage agreements with community based organizations, including local clinics in shelter catchment areas. • coordinate training for social services staff quarterly, which will be tailored to the needs of individual shelter facilities.
