Client Care Coordinator Responsibilities: • Overall delivery and coordination of mental health and related services for homeless families residing in contracted families with children shelters. • Completion of biopsychosocial assessments. • Multi-disciplinary team approach to service provision • Providing information and training to other members of the team around client’s psychosocial stressors and needs to ensure positive outcomes for families. • Familiarity with substance use disorders and addiction treatment services, domestic violence, child abuse and maltreatment, trauma and mental health conditions. • Work from a strengths-based, family-focused, client-centered perspective and identify and fortify the strengths that each family has. • Assist families with specific stressors associated with living in shelter • Ensure that families are receiving full support in shelter and, when necessary, refer families for counseling or other services • Confer and consult with professional and technical personnel in implementing a multidisciplinary approach to client care and well-being • Serve as agency/program liaison to community agencies and/or groups • Develop linkage agreements with community based organizations, including local clinics in shelter catchment areas. • coordinate training for social services staff quarterly, which will be tailored to the needs of individual shelter facilities.
